The Ozarks Health Commission, a coalition of public health entities, hospital partners and community leaders across the Ozarks region, has come together again to better understand the health status, behaviors and needs of residents in our region.
The OHC convenes every three years to reassess the health of our region and issue the Community Health Needs Assessment.
CCHD, in cooperation with the OHC, are asking residents to make their voices heard by completing the health assessment survey: https://form.jotform.com/241124512011031.
This survey takes about 8 to 10 minutes to complete, is available in English and Spanish, and personal information will not be shared or connected with your response.
The primary goal of this survey is to allow decision-makers to have a more holistic and up-to-date picture with which to strategically address community health concerns.
