Women, Infants and Children (WIC)
What to bring for your first WIC visit

In order to enroll you and your family in WIC, we must complete state and federal program information certification paperwork. To help us make that process as smooth as possible for you, please bring with you as much of the following information as possible. If you do not have some of the information on this list, please come to your appointment anyway!

  • Person being certified - The pregnant woman, new mother and infant, or child being certified must come for the visit.
  • Proof of legal name - For an infant you may bring hospital discharge papers or nursery bracelet. You should provide birth certificates for children.
  • Photo ID - Adults should present a photo identification, such as a driver's license.
  • Proof of address - You should bring a utility bill, telephone bill or some other letter addressed to you.
  • Proof of pregnancy - You should bring laboratory proof that you are pregnant. We can also provide pregnancy testing at no cost.
  • Social security cards - If you have them, you should provide social security cards for each person being certified.
  • Medicaid card - If you have this service, you should provide a Medicaid card for each person being certified.
  • MC+ insurance card - If you have this service, you should provide an MC+ card for each covered person being certified.
  • Proof of income - You should bring a pay stub, direct deposit slip, Social Security payment stub, etc. Your gross income must fall into the WIC guidelines mentioned on our WIC Income Guidelines page.
  • Child or infant's immunization records - If you have a record. We provide childhood immunizations at no cost at our WIC clinic.
  • Food history - Please take time to write down a list of what each person eats in an average day and week. You will be asked for the list before you can visit with the Nutritionist.
Please allow two hours for your first appointment. If you are attempting to certify more than one person, allow additional time beyond the two hours.
